Registration Agreement By submitting this form, I understand that: * Registration is for the full six-class term unless short-term Math Support is approved in writing by the student’s teacher. * Tuition must be paid in full before the first class of the term. * My child’s requested time is not guaranteed until confirmed by OCA. * Missed classes are nonrefundable and are not eligible for makeup classes unless the class is canceled by OCA. * Competition Math placement requires a teacher recommendation and successful completion of the placement exam.
